• Download Broker Carrier Agreement. 
• Proof of US DOT Operating Authority.
• Certificate of Insurance showing Transport Logix, Inc. as additional insured. Must meet minimum coverage limits of…

    • Auto Liability: $2,000,000
    • Cargo Insurance: $200,000
    • Excess Liability/Umbrella: $4,000,000 (hazmat carriers)
• W-9 Certificate
• HazMat Certificate (if applicable)
